I give a 5 for all now but in the beginning it was quite confusing regarding medication administration. They kept messing up and could have potentially caused a medical emergency. - change in staff w/o notify family, but new staff are skilled, courteous and kind to my mom - poor communication among nursing staff (NPs - Nurse- Medtech) It's been 9 mos and the PCPs & NO had a change - I still haven't met the second NO assisted by the PCP. The different shift nurses aren't kept within the loop. Mom feel and went to hospital, was discharged and I was surprised the shifts didn't know about her fall. Also mom was acting out of character, I had to tell the NO and she also didn't know about it (she was on vacation) until a week later and then was put on an antibiotic (no urine collected) but could have had a UTI which is why she was acting out. She is much better. If I didn't go see my mom 2-4 times a week. I wouldn't know. I still rate the facility a 5 because the caregiving staff and director is kin and works with me when I do have an issue. Mom likes the place

This community is the perfect size! one story and has a beautiful outdoor area in the center of the community. Everyone was very nice and the residents looked happy. I was pleasantly surprised by the tour. I would definitely consider this place

We thought Brookdale Champion Oaks was excellent. The staff were welcoming and really cared about the residents. I would absolutely recommend this facility to other families looking.

We chose Brookdale Champion Oaks because it was the most convenient so that I could be really involved. Also the staff when I came to talk with them were very friendly, even the residents were telling me how nice of a place it is. There are no kitchenettes in the rooms, so it is a bit cheaper than some of the other Brookdale facilities. They are able to have refrigerators though. It would be nice if in the beginning they provided more information on the levels of care and the pricing, the first bill was defintitely a big shock. I would recommend Brookdale Champion Oaks to others.

It has been a great experience so far. They are attentive to my loved ones medical needs. They have a lot of activities, and my loved one is getting hot meals.
Based on the cost it, I feel like we have had a good experience. Now if you asked my mom she would give you a whole different story. She says things are not great, food etc. But she's just complaining because she's there, not necessarily because I've truly found any major infractions that are worth noting.

I had a tour of the facility while looking for a place for my father, and this was a very clean and friendly place. I felt very welcomed when touring the facility, all the staff said hi and had smiles on their faces.

We have been very happy with our experience. Brookdale Champion Oaks was definitely the best fit for moms needs and her income. They also had just enough activities for her since she is not overly social . She was able to get a private room and bathroom in her budget which was very important to her. I would recommend to others looking.

Our virtual tour at Brookdale Champion Oaks was great. Due to Covid, it wasn’t easy to come in. The manager did an outstanding job on the video tour, and she answered all of my questions. It felt just like being there. Later on I followed up with an in-person tour with my loved one. We were very satisfied. It ended up not being the place we wanted, but the manager happily recommended another Brookdale location, which he moved into and has been a good fit.
Ever since my sister has been at the facility she has been well taken care of. I would highly recommend Brookdale

I have a family member that lives at this facility, and I feel welcomed whenever I come to see my grandmother. The staff are friendly and polite, my grandmothers room always looks tidy and clean. My grandmother has told me that this place feels like "home" to her compared to where she was living prior.

I love the majority of the staff members, one in particular loves my mom and takes really good care of her. There are times I have been there and I've swept the room. I've had to have them spray for bugs as I do find bugs on occasion. I do try to be understanding as I know that it is hard to find good help to work in a facility such as this, but most of the staff members seem to enjoy their job and to take pride in it. My mom seems to like it there and has made friends and wants to introduce me to all of them but I am not allowed to roam the halls at this time because of the pandemic. I totally understand that and honestly I don't want to roam the halls. I think it is great all the activities that the Community Activities Director plans and does with the residents. I just wish my mom would participate more than she does.

The community itself was kept very clean, however our loved ones room was not as clean as we would of liked for it to be. The communication was excellent, they were always in contact with us about our loved one.

The community is very clean. I would recommend this community to others for their loved ones. The food was good, and my loved one enjoyed having family eat with them. The room was good for my loved one during their stay.

Do not place your loved one here. If you do, be prepared to step in when they let your loved one run out of critical medication, don’t clean linens or care about hygiene, leave medication in the residents room, and falsify documentation stating they administered medication at the same time video footage and police reports clearly showed your loved one wandered off while under their care and was 2 miles away from the facility.

Do not place your loved one here. If you do, be prepared to step in when they let your loved one run out of medication, don’t clean rooms or wash linens, don’t bathe your loved one or care about their hygiene, leave medication in their rooms, and falsify documentation stating that they administered medication at the same time they allowed your loved one to wander 2 miles away, according to video footage and police reports.
