Autumn Leaves

This is a wonderful establishment that has many recreation and resident inclusive activities that provide a safe and stimulating environment for the residents that live within the establishment. I highly recommend Autumn Leaves.

My loved one is currently a resident at Autumn Leaves and everything about his experience at this facility has been great so far. The staff is great. The food is good. The housekeeping is great. The care services and activities are great. The value for cost is also great. I would definitely recommend Autumn Leaves to others.

It has some positives and some negatives. Has some cleanliness issues. Not a fan of the doctors that they have in house. We picked this community because of the price, location, and availability.

The nurse has called me to get prescription related items after I set up everything with their preferred pharmacy. When it comes to the activities, they do not have enough actives that my mother wants to do. However based on the experience we have had so far, we would recommend Autumn Leaves to other families looking for a place for their loved ones.

The staff in the facility were great. The food was good but not large portions. Getting paperwork done for the long term care was not always easy. My loved one was told when she first got there that she would be able to walk around the grounds. Her dementia got worse and they told her not to walk outside without someone with her anymore. The one bedroom units are really nice and enough room to have most of your things.

I went by, on [date removed] to visit Mom at Autumn Leaves, She was all tired out and could not stay awake so I asked for help in getting her to bed. I left around [date removed], and I received a call from a Hospice Nurse stating Mom had "passed away". I was stunned. Later on the [date removed]. I spoke with the resident Nurse, (NP) he stated the night staff had called him around [date removed], stating Mom was in heavy labored breathing, so he called Hospice in to check on her. The [removed] nurse arrived at 11PM, checked on Mom, then left. Then sometime later was called back in around [date removed]. and pronounced her Deceased. No one from [removed] Hospice nor Autumn Leaves called me, nor even gave a "hint" Mom was dying. The Autumn Leaves Nurse (NP) stated it was [removed] to have called me. Well, I should have been contacted by both companies. Autumn Leaves, failed in the notification that was due to me in the Decline and Death of my Mom.

Enjoy working with the [name removed] and Social Worker. Quick to response to messages and documents requested. Look forward to expanding our services provided at Autumn Leaves, to build on our successful business relationship.

This facility is very nice, warm and inviting. The location is centrally located in a great area of Dallas. The staff are so helpful and professional. I would recommend this facility for those family members needing care.

I love looking out and seeing the beautiful White Rock Lake. Nice staff, I find them to be helpful and friendly. The Director gives his cell phone number to resident and families so people can easily access him for assistance. This is notable.

I recently visited my friend at Autumn Leaves and wanted to share a positive review online. He could not be happier! He says the people there are really nice and the food is almost always very good. Thank you for taking good care of him.

Staff members are always very professional and willing to go out of their way to help residents and families. . Kind, friendly and caring atmosphere. Excellent location for anyone looking for an East Dallas location.

They provide great care! Great staff and administration. Clean facility. I would suggest anyone to tour this facility! Great location and proximity to downtown dallas. Beautiful views of White Rock lake!

I have had such a wonderful experience with the staff here. They have been attentive and very responsive. The DON and Social worker have been amazing to work with and go above and beyond for their patients and residents. I highly recommend!

It was bought by somebody new, but it appears the folks are doing their best but sometimes there seems to be a lack of communication with the families. That can be frustrating when you do not live in the same city as Autumn Leaves. They do a good job at calling me, but if I call back it is hard to get someone on the phone. Overall I have been satisfied and they have been accommodating. It is a very good value for the money especially in the area.

The hallways and elevators are not cleaned properly. There is a foul order in the hallways.

My 91 year old dad spent almost a month at Autumn Leaves for mobility support after a fall at home. He received great therapy while there and the team of care providers were professional and compassionate. [name removed], in particular, went above and beyond and gave us so much support. We are grateful for the care he received. My only suggestion is that the facility could be better maintained. But the staff and experience was positive and we appreciate that.

My mother was in skilled nursing from Oct.27-Nov.23 2021. I had done a good bit of research and Autumn Leaves had gotten excellent scores from Medicare and other non biased sources. They did not disappoint. The facility may not be the shiniest but the cleanliness and professional staff more than made up for it. My family and I came often and saw firsthand the patience and kindness of the staff. The nurses were quick to address Mothers pain and treated her with dignity & respect. Mom had injuries from a fall so physical therapy was essential. The PT staff were excellent & a big part of Moms recovery. The Director of Nursing was available to us to answer any concerns at any hour and that really made our family feel comfortable. It is hard to relinquish care of a loved one to others but I can sincerely say our time at Autumn Leaves was the best possible experience. We are a grateful group!
